Responsibilities for the Home Care / Home Health Visiting Nurse for Lombardi and Long Term Care Patients will include but not limited to:

  • Provide direct patient care to patients residing in the Bronx area under the long term care program.
  • Coordinators of care, follow-up on changes regarding patient status, medications and assessing for services the patient requires.
  • See, plan and document their care / visits including weekend.
  • Responsible for submitting billing information accurately and timely.

Job Requirements 

 

Requirements for the Home Care / Home Health Visiting Nurse for Lombardi and Long Term Care Patients include but are not limited to:

  

  • Current and unrestricted RN nursing license in NY
  • BSN  Degree preferred
  • Candidates must have a minimum two years experience within the past three years in home care / home health visiting
  • Experience: preferably with a LTHHCP (long term care) or Lombardi program
  • Must be experienced in OASIS documentation
  • Must be eligible to work in the United States
  • Availability: Weekend availability in addition to a few shifts during the week.
