 Responsibilities for Home Health / Home Care Intake Nurses include but are not limited to:

 

  • Evaluating patients referred to home care from the hospital’s Emergency Department (ED)
  • planning, writing and coordinating referrals for home care services
  • collaborating with the hospital’s ED case managers
  • discharge planning
  • utilization review
  • insurance case mangement to facilitate safe discharge
  • completing intake referral information including intake data, essential background information, insurance verification, hospital course and plan of care
  • maintaining relationships with hospital personnel, providing information, and marketing agency services

Job Requirements 

 

Requirements for Home Health / Home Care Intake Nurses include but are not limited to:
 

  • Current and unrestricted RN nursing license in NY
  • Candidates must have a minimum of two years strong home care experience
  • BSN Degree - Master's preferred
  • Must be experienced in OASIS documentation
  • Must be eligible to work in the United States
  • Should be available to work 2-3 shifts during the week in addition to the weekend. Shifts are: 8a-4p & 4p-12p
