The RN Case Manager makes the initial evaluation visit, regularly reevaluates the patient’s nursing needs, initiates the plan of care and necessary revisions, and furnishes those services requiring substantial and specialized nursing skill. This position initiates appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the patient’s condition and needs and counsels the patient and family in meeting nursing and related needs. The RN Case Manager participates in in-service programs, and supervises and teaches other nursing personnel. Travel 100% of the time.
Must have an active Indiana RN license in good standing and at least one year of previous nursing experience. Home Health or Hospice experience is preferred. Must have knowledge of the use of personal computers. Must have reliable transportation and a current Indiana driver's license. Must be able to handle frequent scheduling changes and make independent decisions.
