We are seeking a knowledgeable and empathetic Hospice Chaplain to provide pastoral care interventions such as spiritual and emotional counseling, performance of sacerdotal functions and religious rituals and conducting religious services that are determined by spiritual assessments, specific settings and sensitivity to a variety of values, beliefs and religious orientations.
Additional tasks of the Hospice Chaplain include:
- Visiting patients and their families upon admission and receipt of a Pastoral Care Notification to provide emotional and spiritual guidance and counseling
- Performing spiritual assessments and integrating them with each patients’ interdisciplinary team plan of care
- Performing religious sacraments and rituals at the request of patients, families or home churches
Hospice Chaplain – Religious and Spiritual Counselor - Minister
Job Responsibilities
As a Hospice Chaplain with Community Home Care & Hospice (CHCH), you will be a core member of our interdisciplinary team as you provide pastoral care services that integrate with each patient’s total care plan.
Additional responsibilities of the Hospice Chaplain include:
- Providing pastoral and bereavement services such as visits and phone calls upon the immediate death of a patient as needed
- Coordinating with the Hospice Social Worker and Bereavement Representative in planning, conducting and attending memorial and funeral services as appropriate
- Acting as a liaison between patients, their families, local clergy and our interdisciplinary hospice care team
Hospice Chaplain – Religious and Spiritual Counselor - Minister
As a Hospice Chaplain with CHCH, you must be in good standing with and have the endorsement of your faith body. Our ideal Hospice Chaplain seeks excellence in pastoral care through an authentic demeanor and skill with the spiritual dynamics of illness, loss and death.
Additional requirements for the Hospice Chaplain include:
- Bachelor’s degree in Religion, Biblical Studies or a related subject from an accredited institution with a minimum 2 years pastoral experience
- OR Associate’s degree in Religion, Biblical Studies or a related subject from an accredited institution with a minimum 5 years pastoral experience, 2 of those years in clinical pastoral experience preferred
- Ordination or commission to act as a minister of pastoral care
