POSITION SUMMARY:


The Speech Language Pathologist (SLP) is a professional member of the health care team who provides skilled speech therapy services to patients. The care given will be within the scope of the therapist's education and experience; this will also be in compliance with physical therapy standards of practice, federal, state and accreditation regulations.

MAIN DUTIES AND RESPONSIBILITIES (Essential Functions of the Position):

Assesses and evaluates patients and establishes appropriate plan of care in accordance with the physician based on realistic functional treatment goals.

Provides direct patient treatments with the Speech Language Pathologist scope of practice with the goal of restoring function and maximizing patient performance.

Observes, records and reports the patient’s response to treatment and any adverse changes in the patient’s condition to the physician and the clinical supervisor.

Instructs and advises patients, families/caregivers and other health team members as appropriate to assist in reaching the goals of the plan of care and maintaining patient confidentiality.

In the absence of nursing, supervises and instructs the home health aide as needed. At lease every 14 days, assesses the patient’s need for unskilled care and revising the plan of care as appropriate.

Completes documentation as required and submits per agency policy.

Attends agency orientation, meetings, case conferences and in service as scheduled.

Obtains authorization for treatment as required.

Coordinates the plan of care with the physician, patient and other disciplines through verbal or written communication.

Calls patients prior to scheduled visits.

Responsible for keeping current in speech language pathology trends and knowledge.

Performs other duties as required. 

 

 

 RequirementsQUALIFICATION GUIDELINES
EXPERIENCE/TRAINING/EDUCATION/CERTIFICATION:

Required:

Master’s degree with a major in speech pathology.
Must possess a license issued by the California Board of Medical Quality Assurance and a current BLS-CPR certification.
Meets the education and experience requirements for a Certificate of Clinical Competence (in speech pathology or audiology) granted by the American Speech-Language Hearing Association; or
Meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certification.
Must have one year recent healthcare experience as a speech therapist.
Must be self-directed and able to work with little supervision and have appropriate clinical judgment and communication skills.
Able to demonstrate knowledge and skills necessary to provide care primarily to the adult and geriatric populations served.
Must have current California driver's license plus proof of car insurance.
Requires the ability to establish and maintain a courteous and cooperative (service-oriented) working relationship with patients, families, customers and co-workers.
Requires the ability to fluently speak, read, write and understand the English language.
Must be able to perform the physical requirements of the job.

KNOWLEDGE:
Ability to work independently.
Knowledge of home health preferred.
Good communication skills.

Travel: Position requires minimal business travel and a valid Driver’s License.

Physical Demand: Light physical effort. For example, standing, bending, or stooping for extended periods, operating light office equipment, e.g., personal computer, copier, fax machine, etc., manually handling medium weight materials and/or equipment (0 to 10 pounds).

Dexterity: Light dexterity, e.g. using data processing equipment, long periods of standing/sitting, etc.

Mental Demand: Moderate to high degree of concentration due to volume, complexity, and/or “pressure" of work.

Working Conditions: Clean working conditions with no disagreeable factors.
